If you ever have an emergency, please make it clear to our staff, and they will immediately do what is necessary.
Please arrive 10 minutes early to allow time for check-in procedures (15 minutes if you are a new patient).
Kindly give 24 hours’ notice to cancel or change appointments.
Please bring insurance information and identification to each office visit.
A parent or legal guardian must accompany children under 18 years of age.
Please bring to every visit, a current list of all your prescriptions, over-the-counter medications, supplements and remedies.
Patients will be charged for no-show appointments or appointments that are cancelled with less than 24 hours notice.
Please allow 24 hours for prescription refills to be processed during weekdays.
Diagnosis and treatment plans can not be appropriately made over the phone, E-mail, or fax. Therefore, we can not call in prescriptions unless you have been seen by our professional staff. |
